Farmington/Farmington residents to be part of local health survey
Farmington and Farmington Hills residents are invited to participate in a Community Health Needs Assessment Survey by the Farmington/FH Health Advisory Council. The information will assist communities inclusive of Botsford Hospital, the school system, faith-based groups, local employers, and municipalities to learn how best to support the health of residents in the future.
The survey is being conducted as part of the federal government's Accountable Care Act requirements for all non-profit hospitals to initiate in their selected communities.
The survey is completely voluntary and answers are anonymous. The survey instrument is now open online now through July 19 and takes only 5 to 10 minutes. To obtain the on-line survey go to https://www.surveymonkey.com/s/FarmingtonAreaCHNA.
If residents prefer a printed survey with postage paid return, call Botsford Hospital Community Relations at 248-442-7986 to receive a printed copy by mail.
Results of the survey will made public in the fall.
