Advance Care Planning
discusses the importance of advance care planning and helps individuals
initiate important advance care planning conversations with loved ones.
Attendees are introduced to ways advance directives can be shared with health
providers, and familiarized with numerous resources for initiating
conversation, creating an action plan and completing an advance directive.
